PROGRAM OVERVIEW
The focus of the Locke Lord Wellness program is to provide an increased incentive to take care of ourselves by understanding our health risks and taking preventive steps to stay healthy. With the assistance of this program, we can become motivated and be in control of our health and wellness.

The Wellness program is available to all Locke Lord employees and Partners, and is geared towards encouraging you to take control of your health and then providing you with a reward for completion. Participation in the wellness program is strictly voluntary.

PROGRAM DETAILS

Annual Preventive Physical

Participation and awareness are two important components of how we take care of ourselves.  The steps we take to prevent and/or identify illness and disease in the early stages and the changes we make to correct and/or improve our overall health are vital to our well-being.  Obtaining an annual physical exam with your primary care physician is one step we all can take to get a baseline of our general health and help our physician detect and treat any health issues early.  As an incentive to receiving your annual preventive exam, the Firm will provide you with $50!  Why wouldn’t you want to do something good for yourself and also get paid for doing it?

No Cost to You!

The Locke Lord medical plan options offer an annual preventive exam benefit with 100% coverage when you use in-network providers.  Therefore, your out-of-pocket expense for obtaining an annual physical exam will be $0!

If you are enrolled in the Locke Lord medical plans, BCBSIL will provide the Benefits Department with verification that you have completed your annual exam.  Once we do, we will provide you with $50.  If you are not enrolled in the Locke Lord medical plans, you and your physician will be required to complete the Annual Preventative Physical Verification Form and return it to the Benefits Department in Providence/27 E.  The form is available in the Resources section above.  Be sure to take the form with you when you go for your annual exam!

Health Assessment
The Health Assessment is an online questionnaire from Blue Cross Blue Shield of Illinois (Well onTarget) that requires you to complete information on your health history and lifestyle, including stress, exercise, eating habits, alcohol use, safety practices, and readiness for improvement. Once you complete the Health Assessment, you will receive your Personal Health Report to help you better understand your reported lifestyle habits. The report will include helpful suggestions for how to improve your lifestyle based on your profile. The Health Assessment is not intended to be a substitute for or replace any medical care or advice from a physician.

The more honest the answers you provide, the more helpful your report will be. The Health Assessment will take you approximately 15 minutes to complete.

Please Note: All Employees and Partners not enrolled in the Firm’s BCBSIL medical plans have access to the Well onTarget site.  Your personal credentials are mailed to your home address via a postcard which will allow you to create a Well onTarget account.  If you cannot locate your credentials, please contact the HR Hotline.

Why Should I Participate?
Most employees do not readily have the time or access to the information they need to monitor their health. Your Health Assessment results will indicate your health risks, and help you to modify your lifestyle. Awareness plays a huge role in health improvement and Locke Lord wants you to have all the information you need to live a full and healthy life.

How do I access the Health Assessment?

New to Well onTarget?

1. Log on to www.wellontarget.com
2. Under New Member, Click “Register Now.”  Follow the instructions to complete the site registration. You will need your BCBSIL member number.
3. Once you hit submit, you’ll be taken to the Well onTarget homepage.  The Health Assessment is acessible under the My Health Tools in the top navigation bar, the image slider, or is the first step in the Start Here widget.
4. Upon completion of the Health Assessment, review your detailed report and overall wellness score. Click on Take Action Now to get a list of recommended tools and resources (including workshops) based on identified risks.
5. Select your personal health improvement plans and use the Well onTarget resources that most appeal to you.

Already Registered with Well onTarget?

1. Log on to www.wellontarget.com and enter your username and password.
2. The Health Assessment is acessible under the My Health Tools in the top navigation bar, the image slider, or is the first step in the Start Here widget.
3. Upon completion of the Health Assessment, review your detailed report and overall wellness score. Click on Take Action Now to get a list of recommended tools and resources (including workshops) based on identified risks.
4. Select your personal health improvement plans and use the ahealthyme resources that most appeal to you.

Completely Confidential
The information from your Health Assessment is completely confidential. Locke Lord will not see or have access to your individual Health Assessment information.

Reward Yourself
Once you complete the Health Assessment, you earn life points which can be redeemed in the online shopping mall for gifts from the merchant of your choice.  At the end of your assessment complete the mailing information details and your gift will be mailed directly to your home address.
